Flags are not added casually. Reports are investigated, evidence is reviewed, and recommendations go through senior review before a decision is recorded.

How a report becomes a decision

Every report follows a defined path from initial review to a recorded outcome.

    A report is sent with relevant information and evidence.

    Review

    The report is checked for validity and priority.

    Assign

    A staff member claims or is assigned the report.

    Investigate

    Evidence and context are reviewed in full.

    Recommend

    The investigator records findings and a recommendation.

    Senior review

    A senior staff member reviews the recommendation.

    Decision

    A final outcome is recorded.
